1. After filling out the Contact Information Google Form...

2. Choose your size

3. List your Full Name, Phone Number (xxx)xxx-xxxx, and Email address the same as you did on the Google Form.

4. Once you "Add to Cart", you will be able to adjust the quantity of each item.

5. If you would like to purchase multiple sizes, you may select "Continue Shopping" in the top right hand corner of the page and it will re-direct you to this screen to add more items to your cart.
