| 2 | Parents, the following forms need to be downloaded, filled out, and printed, so that you can have them notarized before remitting them to us.
- Click the name of the form to download it, and then open it.
- The form should be filled out on your computer by typing the information into the fields provided.
- Fill out all required fields and double check that the information is correct.
- Click the "Print" button to print the forms.
- DO NOT sign the forms until you are in front of a notary.

| 3 | Please get these forms notarized. Also please make sure that you make a copy of your insurance card (both front and back of card). The copy of the insurance card will need to be turned in with all the other paperwork.

| 4 | Students should schedule a physical for the upcoming school year. School sponsored physicals are given on Wednesday April 13th, at Ola High School from 3:35 to 6:30pm. The cost of those physicals are $10 per student. Physcial Documentation will not need to be turned in until the start of band camp!!
